It's either a bug or you keep your config open after sending it to the IPOffice.
If your IP-extensionnr was 'autocreated' after getting the first config from the IPOffice every merge you'll send back a config without this IPExtension. So the phone reboots and gets autocreated again.
I would check SSA and see if the IPOffice hasn't restarted. I had a customer with the same complaint.
