I am sure you have probably done this as a first option, but a restart of the system has resolved a couple of issues for us on Embeded VM.
Fortunatly this is few and far between but worth a go.

We always recommend checking the bin files in the Admin CD/download as they normally always include a .99 version if required and from the file name you can ascertain if this is relevant to your system.

We have exactly the same with IP Dect on 4.2.x.
We have good local latancey etc but we are running over sip trunks and believe we have some lazy lines on the trunk side but we are still not convinced as it only seems to effect the IP Dect.
We have done a lot of work with SIP Trunks...

We have found the following vital in configuring SIP:
1) Running Stun - even if it doesnt do much but define the firewall or router.
2) Set the registration expiry to below the NAT timeout - we use 60, which creates traffic but resolved some issues.

If you have the router already then the router is the obvouse choice.
You will need two lan ports in the router, address them as two separate segments and then make sure that each device on each segment has their gateway set as the closest lan adapter address on the router.

The are a host of idle timeouts - fast idle timer, idle timer, renable timer, carrier timout but, they still don't help the specific problem of packets not being intresting afer the line has just timed out!

Do you have any NT servers or workstations attached to the network that the router is attached to.
NT boxes are realy noisy! However, there is some good documentation of using access lists to make sure that the unwanted traffic is not intressing and thus does not bring up the link!
