try checking if the extension is already active or used. Try command "sta sta 2495". If it is on serv-onhook, it means it is already registered or somebody is already using that extension.

Have you tried activating the station lock feature? this can prevent stations features being used by others even if you moved them physically to a different location.
are you using an IP phone in dialing the conf brdg number? I do experience this also; tried changing a parameter on the Ip phone (801.q something) to on rather than auto. changed the eth int to 100MB full duplex also. its now working.
if only 5 ip softphone on one group only, then best way is to let hem log themselves the extension with password. in this manner, you can restrict any of them using more than 5 licenses. email them the extension and corresponding password before the start of the shift. :)
setting up ISDN E1 as connection to telco provider is difficult. If your HW test loop back works fine, then it is a telco to PBX config problem. try asking for the configuration settings on the telco side, maybe you can see the config conflict.
