I think that the only way to default a DO is to get Avaya to dial into it using their inads login and change the password. The console port is locked down so that only Avaya can get onto it. You could try downloading the latest DO software from the Avaya support web site. Place it on a USB stick...
You do not connect to the Audix from the prologix. There should be a seperate spider cable attached to the back of the system adjacent to the slot were the Audix is located.
I think you are looking at the wrong vector with regards to these events. The entry 4/12 and 4/13 in event data 1 refers to vector 4 steps 12 and 13. Stinneys comments are valid however. I would run a test call whist running a trace from your extension to see what the issue is.
If you look at the back of the system cabinet, there should be a comms cable going from a port marked Terminal. If there is, it should go to the terminal. You should be able to use that to link to your PC. Wiring depends on if you are in the UK or not.

If you have the root login for the CMS, carry out a trace route from the # promp to the phone system and see where it stops. I think it may be a default gateway issue.
Have you pressed the on button at the bottom left hand side of the server using a pen. For more info there should be an alert label on the top of the server

The latest version of VAL firmware unfortunately only works fully with version 5.2 ASA/VAL manager. One of the issues is that you cannot see these boards as audio sources.
