Are you calling the the site number in the app from site? PRI trunk to trunk transfer might be blocked by the telco. Try putting the number in the app and cAlling from from your cell.

1st. CLID needs to be enabled at the CO. It can be verified by using a POTS line phone with Caller ID capable. Caller ID is transmitted between the 1st and 2nd ring.
2nd. CLID needs to be turned on for each line in LINES > TELCO FEATURES
3rd. The phones you want to display Caller ID...
Is it possible the 1140e has a vlan set in it's config? This will stop connection, but will allow S1 and S2 to be populated and the the set to geet an IP address.
Scroll through the entire terminal setup the vlan setting is near the end.

Try this:
1. Once connected to the console of the device using OneAccess console cable. Press <ESCAPE>, then <CTRL>+Y and finally <CTRL>+N.
2. You will be prompted whether you want to factory reset the device. Type 'Y' for yes.
3. You will then be asked if you would like to reboot the router...
Typically cables are punch onto the back of a BIX1A and the feed to the back of a 5A or 7A. Then you cross connect from the the 5A front to the cables on the 1A. If you need more than 5 connections you can bridge the 5A sections together on the back.

not sure on the number of intercom buttons - would that restrict ability to xfer?
Absolutely. You need a free I/C path to transfer. So if your calls appear on and I/C button you need a 2nd I/C to make a transfer. If call is on a line button only 1 I/C is needed on the phone.

The splash ring you are hearing is caused by Delayed Ring Transfer (DRT). It occurs when no phones are programmed to ring. By default the DRT is set to PRIME. You'll fine the PRIME set for each line under LINES programming, change them to NONE. Alternately you can change DRT from PRIME to NONE...
You can "hear" the disconnect from the CO. Put a butt set on the line in monitor mode. A few seconds after the far ends hangs up you will hear a buzz as the disconnect signal is sent from the CO.
