I turned on logging of traffic on the system side for all allowed and blocked traffic. I am not seeing the RTP packets hit the firewall/router at all. I can try to forward all traffic from the external IP regardless of ports and see if that helps but that is not a 'proper' fix.

You can force the sync be restarting the voicemail service or you may have to wait.
