    Before any upgrade to release 5.0 and up get the 2nd level password 1st. If you can not get them, after the upgrade load the software again and choose utility option then restore the default password. This will restore both level 1 and 2 back to 0000 (4-zeros).

    I have same issue at one of the customer, i re-transmit everything to those bad site/ITGCards and then reboot them. They work after the reboot.
