Outlook 2010 is not yet supported. however there was a similar situation with 2007 in where Microsoft Exchange Server System Manager Tools were the cause of Outlook crashing. Un-install the MS System Manager Tools from the Client PC. Not sure if this holds true for 2010.
you can either use a pc as a FTP server and change the provisioning IP address of the phones and download updated firmware for the phones, or install SU28 that was released on 8/13
you can use the BIT tool to reimage the systems to whatever release they are on now which will wipe out all the programming. And if you have a backup you can restore the database with the backup
yeah same here. Uploaded the files started applying and then got disconnected. waited an hour no reboot logged in no alarms of the upgrade failing. maintenance window expired = round 2.

sounds like Telco does not like the clid info the system is passing thru the circuit and causing a disconnect.
May want to add a Public OLI entry for the dns assigned to vm. and have telco trap the calls going across the circuit to see what is actually being sent.
is the BCM located at the main site? what happens to a pc if it is not connected to the back of the phone but directly connected to the network switch?
