With only 4 users remotely, depending on the application access required, you could consider not having a second server at all - create a network to network VPN and have your client machines authenticate to the SBS...
I followed the Microsoft documentation for the conversion from SBS200 to SBS2003 and in both instances hit massive issues that were eventually reslved by performeing a brand new install.
This was before the swing migration tool was available...
I cannot recommend more strongly that you avoid...
SBS2003 has to be the domain root.
Depending on the number of users and complexity, it is sometimes easier just to wipe out the SBS2003 environmet totally and reinstall from fresh.
ONLY viable if you have relatively few users -Less than 15 or so is probably managable - no special security...
Correct inasmuch as you need to purchase separate license for the additional server and SQL server - but client access licenses are covered by SBS2003 R2 Premium.

Not certain what the challenge is here...
It appears that you have VPN links set up to enable application access from the branch offices to the head office.
To enable them to access the internet via the head office, simply add a route to the internet via the network address of your head office...
The Linksys option is probably your best bet... but you will almost certainly have to replace all of your equipment (I assume you have Symantec Gateway 100/200 at each location) as I would anticipate major headaches trying to get a VPN connecting between that and the Linksys.
Use the DynDNS...
It also appears that you might need to look quite closely at your network architecture...
Do you have load balancing on the cable and DSL at the primary office, or failover? If failover, you might want to consider splitting the VPN traffic and having half (or even more) on your failover...
