I started browsing & tracked down a couple of places selling the InTools plugin for CS & CS2 - ranging from $15US to 59Pounds. The latter has a demo version available for PC or Mac but I could only install it on InDesign CS - maybe CS2 is available once you buy it.
Easy - Use a paragraph rule, make it the weight of the height of the reverse box you require, colour it to suit your needs, adjust the offset to position it or adjust be offset of the type - sometimes a combination of the 2 works best.

Touch & hold the tab on any one of the toolbars & drag it out into your desktop then drag the others into the new palette. You can drag the others so the tabs line up in the same toolbox or dock them at the bottom.
For one of my jobs, I also write postscript to a specific ppd. I put the ppd in OSX/Library/printers/PPDs and restarted my computer to get it to show up in my list of drivers available to ID.

My Quark 6.1 alerts me whenever a font is missing.
Go into your Quark Preferences & under "Font Mapping" make sure that the "Do not display the "Missing Font" dialogue box is NOT checked off.
