You may not accept a new call when a held call recalls back to you... This is why i was getting the issue after 2, 3, 4 calls. It depended on how fast i called in. It follows the hold recall in consol permissions.

You can also put this on a button for a specific user.
Is there only 1 PRI on the system? If more than one, you have to turn the clocking off on the second. Is the internal csu off? Try to get a monitor trace to see what is happening when the system locks up.
Are you using the 4 pair cable between the phone and the power supply? Pins 7 and 8 carry power to the phone. A single pair cable can be used between the wall and the power supply.

Make sure you have "allow analog trunk to trunk connect" ticked under lines. (if using analog lines) I had a similiar issue where it worked internal until I ticked this.

I used a PCMCIA adapter with a smart media card and it worked fine. I also tried an adapter with a compact flash card and it did not work. I think it matters on how the memory card is formated. The nortel tool works fine as well and does come with some good directions.
