Do you have the install CD? It is on there in one of the folders. You launch it from a windows PC and it makes the floppy. At least this is true for the 4.1 install CD.

I am using it with Outlook Express. My attachents are VBK files, which play with a Nortel supplied player.
I have never been a fan of Outlook, but I might start using it if it does give me a wav file.

When you program that button, was it an internal auto dial? My tech tried it, but he said it still grabbed the copper lines. I didn't think to ask if he was using an external auto dial key.
