In Backup Exec, go to the Devices tab and right click the device that holds the media. Choose catalog from the menu that appears. This should create a new catalog for that media, so the program knows what's on there.

Try removing the line that says NewMailSeqNum=... in the notes.ini file. This is just a counter, removing it will reset it.

Can you let us know what hardware and software you are using? Which OS, which Service Packs, which version of BE, what type of DLT device, ...?
Also, under the advanced tab in the properties of your backup job, do you have the box "eject media after job completes" ticked? What happens...
