There should be 3 SMDR logs from the call made from the IP phone (you are currently showing logs ending with A and C):

Mitelpassion - a "non-SX2000 PDG Member" has been added successfully to the cluster! I set it up just as all other cluster elements, other than I did not select managed by this Ops Manager. This site is then added to the portable directory group, but since managed by this Ops Man is not...

If you check the remote busy lamp assignment form in each pbx, and recall the monitored device, are all of the extensions you have manually entered on one pbx's "remote host set directory number" field appearing on the other pbx's "local host set directory number" field?
You also need to have the remote busy lamp assignment form completed. If anything is not working correctly, you should see maint logs each time a call is placed to the monitored device.

Is it possible that there are ext's at (c) that are forwarded to the 2 users at (v)? Can you go back and read SMDR logs to match the numbers of the 2 ext's at (v)?
I just looked into this a few days ago - On Mitel's website they mention VERINT, VoicePrint, and OAISYS as call logging/recording products to use on Mitels. I haven't had any experience with any of them....yet.
