PCs in VLAN2 do not need IP Helper pointing at servers in the same VLAN2 - those servers, being in the same broadcast segment as the PCs, will see the PC's broadcasted DHCP request - the IP helper does not need to forward the broadcast within the same segment that the broadcast is sent.
Check your "User and Device Attributes" Form on the controller and identify one Device ID from the "failed" list, and one that isn't on the "failed" list.

When you setup the LDAP query in the MiCollab, you can assign AD attributes for phone, second phone, mobile and DID. (Among other things).
The thing you call "CLIP" is I think what Mitel calls CPN - pretty sure you can't upload that from AD, but after you provision all the users, you can upload...
Do you see in the VLAN10 DHCP scope a lease appearing when the server makes the offer?
Does your switch use LLDP to tell the phone about VLAN10?
Have you tried adding the new phone to a switchport that has an old (working) phone in it?

I don't think admin users can be AD accounts, they are local.
The controllers do not use AD, they have their own phonebook. Same with MiCollab.
Where you use AD is in provisioning users out of AD onto MiCollab, which creates the directory for the phone system. You should use email address as well.
Check your Feature Access Codes on the controller for "Message Waiting - Deactivate".
If it is, for example, #33, and the extension with the flashing light is 1234, then dial #331234 to switch it off.
What device are the router addresses on - 192.166.0.1 and 192.165.0.1 ?
What does the controller have configured as its gateway IP address?
What subnet masks are being handed out by the two respective DHCP scopes?
