I just heard on CNN yesterday another twin-word confusion that is like fingernails on the chalkboard for me:
Former Editor-in-Chief of News of the World said:

Holy smokes!...incidences? There is no such word, and an Editor-in-Chief of a newspaper should know better. Incidents such as this should cause journalists to verbally flog colleagues that perpetrate such gaffs.

There seem to be plenty of people who can't grasp the difference between "advice" and "advise".

Epicenter and center (Not exactly twins but misuse of epicenter is becoming rampant)

Epicenter is being commonly used to mean something like "Right in the middle of"

Epicenter actually means "At some point directly above"
