If you rename the file extension from .xlsx to .zip, does it open as an archive with 7-zip or WinRar or whatever you use for zip files? If not, your file is corrupted.
If your company has Adobe Acrobat Pro, you can scan this image and import it there.
Then you can make it a fillable form, by going to Forms...->Start form Wizard...
