This issues was completely related to our SIP trunking provider. We changed to bandwidth.com and all of the issues disappeared.
The problem is that the majority of SIP providers do not respond correctly to the invites that the 3300 sends out when a call is placed on hold.

We have just received new SIP trunks from Bandwidth.com and are having trouble configuring the trunks.
When an incoming call is presented to the 3300 it responds with a 404 not found".
The trunks seem to be set up correctly on the 3300, the only difference I see in the initial invite is...
I have the recall timer set correctly on all COS's, the system sems to pull the call back after only about 5 seconds.
Any ideas what else could be going wrong?

it turned out to be something really stupid, the First alternatives options needed to be set to "This" to accept a DN without giving the error.
Now transfers to VM are happening 100%.

I confirmed this issue as being related to the way that our SIP trunking provider is handling the hold requests from the 3300.
It seems that only certain trunk providers are able to deal with the Mitel requests successfully.

I am struggling to register a remote user via the MBG.
I have created a user on the 3300 (an can connect to it using a VoIP client on the LAN), the issue is I am unable to connect from a remote location via MBG.
I have enabled SIP on the MBG and added the user in SIP Devices. Any idea...
That solves some of the mystery...
The ext has 2 entries, 1 for no answer internal and 1 for no answer external, both pointing to the VM Hunt group and both enabled.
Explains why internal calls are going to VM, but External calls still do not get forwarded.

Will give it a try, yes the VM is on the same system.
I was just thinking it may have something to do with the incoming trunks and signalling not being 100%, but I doubt it.
I'm going to test anyway.
1. Nope I have checked and double checked, no forwarding and Rerouting.
2. It seems that I cannot add any DN to the rerouting form??? Each time I get "The directory number is not allowed”.
Locate Number 8200 shows it as a Hunt Group.
What device is doing the transfer??? - Call from Ext SIP...
OK, think I have it.
The first alternative is set to 2.
2 does not have a DN set in the First Alternatives Forms. When I try set the DN to 8200 (The VM hunt group) I get the error: "The Directory Number is not allowed"
What am I missing?

You are missing the point.
All traffic between the Mitel network is encrypted so you have 1 of 2 options:
1. Disable encryption and try to record the rtp packets directly off the network.
