We found that by changing the T1 format from ESF/B8ZS to D4/AMI the problem goes away.
I'm still only guessing as to why. I can only surmise that we were getting some type of false frame or something.

It sounds as if it's happening to more than one phone.
If one of the phones happens to have keys on it that are in a hunt group then it could also be that the VM is lighting a lamp for the hunt group and the phone with the lamp is the 1st one involved.

Create a phantom ACD Agent.
Create a Phantom ACD group.
Create a Phantom ACD Path.
Interflow the path to VM after 1 second.

I've had a similar issue on a couple of sites.
When I change the route type from "IP/Xnet" to "Direct IP" then suddenly everything works like PFM.
These trunks worked after the upgrade and then no longer do.
No clue why. Reboots do not help.

How many trunks do you have? Is it possible that the cause is you do not have enough trunks to take a call in and send it back out externally? One call will use 2 trunks in that case.

When ready to leave the msg, press hand off key that rings an external recording device. The device answers, plays msg and hangs up.
