I had the same issue on a 500V2. After swapping out all equipment, I finally took the trunk cards out and installed an old IP 400 analog trunk 16. Problem solved, never happened again after that point.
If you use a 3rd party TFTP server and point it to the Manager folder, it will downgrade. For what ever reason, the Manager app will not let it downgrade or atleast not for me.

@fshaw, we had the same problem with MOH. The most used CLEC in this area is Nuvox. It was a release issue in the Cisco IAD. Once they upgrade the firmware, it was magically fixed.
