I don't have OTM, but don't forget there is also a CLID prompt in LD 15. Post the CDB and RDB printout from Ld 21. Hopefully this will trigger other comments.
If these are calls to a non-call centre private number on his phone then 1 solution is to use remote notification on that number to a personal pager. If he's on a call & someone leaves a msg his pager goes off. If he's walking around etc he may hear multiple pages.
If this is for an ACD number...
Wait 8 seconds after dialing is completed - I always do this....or
...immediately try entering a <CR> and see if a response is sent
...also try different speeds in your setup...

2)does the call re-establish itself after the dropout or is it disconnected ?
BUG messages are generated by software; for illustration only, eg - a set goes on hook that was established, but before the software sets that bit to onhook, the programmer makes the software check that bit to ensure it is not onhook already. If the bit shows eg the set is idle - which is not...
Step 1 - hate to be obvious, but advise your users /management of planned shutdown time, duration of outage, restore time and any call redirection of incoming calls that you may invoke.
Final Words: As of Dec 1/06 the GA release for this problem with dual language prompting is still being planned. Nortel apparently has been requiring colocating all Mgate cards (a12ozbottle is correct on this) for a few months. This dual language prompting problem originated in the CP buffer...
