If you're wanting easy, there are several free services out there that offer polling/surveys - Bravenet being the first that comes to mind (http://www.bravenet.com). B-)

That way works fine, or you can go into the code view and use the Insert Layer command from the menu twice. It should automatically set the second one inside the first that way.

Actually, you can go into the Edit/Preferences and select the "Code Format" section. In there you can specify the case you want for your tags.

OK - if I'm understanding this correctly, your nav menu is on the left and you want it to stay at the top. Looking at your code, you simply need to change the vertical alignment for the cell the nav image is in. You currently have it set to "middle" - if you set it to "top"...
You can also go to the "Publish Settings" and, on the HTML tabe, adjust the dimensions...setting it to match the dimensions of your movie, to exact pixels (other than the movie default size) or set it to percents (100% for example).
