On the 1408 phones, when a call comes in, the entire CID is shown on the display - the office uses this to check medical records.

The issue was that the system automatically created new blank extensions, so they weren't technically "logged out". Once I deleted those "placeholder" extensions, the phones correctly showed "Not Logged In" along with the login button.

The problem ended up being exactly as suggested - the extension was assigned to mailbox 10 as the general delivery. I removed the assignment, and it then allowed me to assign to Ext 54.
