you need to track based on your hdiskx (x is hdisk number on VIO & mapping i.e output of lsmap -all or " lsmap -vadapter vhost0" say for vhost0 you have 5 disks. , from that you know disk's number & LPAR to which it associated.

Are you using logical volumes in vg to assign to new lpar or assiging disk to vio then assigning that disk to lpar ?
If you are using hdisk on VIO then assiging to lpar you can assign what ever diskx you want to assign to client.

As far as I know it should NOT have any impact on Client Lpar'S, when you change hostname name of VIO Server.
It's seems like all vscsi mapped is only related by slot number's .... etc. I don't see anywhere it's hardcoded with hostname.

Which version of HMC you are running ? ,Is that you have same HMC for P5 & P6 ? ,If you have different HMC for P5 ,I would check hmc LAN Adapter settings for particular Interface where you have issue.

Did you verified the hostname is listed on correct ethernet interface i.e eno, en1 ..etc. what ever ethernet interface you are using.

verify for same ehternet you have hostname entry.
