Same problem here. NDRs just occasionally. No rhyme or reason. And mine can send to the same address at a different time, just not when the problem happens.
I have had the same issue for about a month.
Really strange one. Very sporadic.
- about 5 people of 300 on 1 of 2 ex servers
- all 5 are in 1 building but there are about 100 other people in the same building w/ no problems.

All I have read and heard also points toward not running defrag unless app log states it is needed. White space is generally thought to be used faster than free space by exchange since exchange already knows it is there.

Make sure you typed the reg files EXACTLY like they show. I had an extra space in the Per-Site entry and it did not work. Also use the admin -r and get the Dir name of the mailbox or CR. Check out MS article Q239427 also.
I have the same thing, blank originator in outbound queue and I would like to stop them also. I have relaying blocked but these are emails that come in for a non-existant user at your domain. The user name is invalid but the domain is valid.

Along with that I would not have any users mailboxes with no limits. If they get a heavy spam or repeat delivery of an email it could eat up your free space quick.
I would set up that user with a really big mailbox, but never just unlimited.
I had the same situation archiving/ moving older emails in the active mailbox with attachments.
I enabled background scanning of attachments on NAV for Exchange and the problem went away.
If you just want to see how many emails are traveling in and out(via internet)you can use SMTP logging and open the log in excel. Then you can sort by sender, or whatever you want. Just select minimal and it will give you a count and some other info.

I had the same error message recently, although mine was with archiving. It turned out NAV for Exchange server was locking messages with attachments so you couldn't move them.
I enabled "Scan attachments in background" on NAV and the next day all was OK.
