You have the right idea. The only thing to remember on a combo card is the port order. Ports 1-6 are digital stations, port 7&8 and analog stations, and ports 9-12 are analog trunks. If you look at the sticker on the front of the card it will show you this info as well.

Keep alive was enabled. I think I found the issue, SIP ALG was not disabled on our Cisco ASA. Disabling that seems to have fixed the issue, although I have to do some more testing to make sure. Thanks for your help!

According to the IP Office matrix the mobile worker license should be all you need for the One-x Mobile client. Is that not the case? Or is there limited functionality with just the mobile worker?
I believe a yellow alarm indicates a transmission problem and has to be received from the circuit provider. Swap the cable and then push the provider harder.
