I think you still experience some 1 way audio conditions. The Mitel MBG (Teleworker) application has greatly reduced in price and would be a good solution for you. For a good SIP client I would look at www.counterpath.com
You have multiple options if you are have a Mitel 3300. If you are running on an SX-2000 then are limited. Depending on the application UCA might work but be careful as it doesn't have full TAPI support. Mitel calls it talk to TAPI and many applications cannot open the driver.
This typically is a carrier issue. Can you manipulate digits with no issue such as pulsing your DID #? If that works just fine I might be able to point you at a couple different areas.
Are you refering to UCA3.0 or 2.0? How exactly is it showing off line, the chat? I have installed several customers on the 3.0 without any issues. Is the client trying to connect via IP address or FQDN? I prefer FQDN.
