If you are setting a user to forward offsite make sure that you assign an extension to the user as well. I used an analog extension then remote forwarding worked for the user i created.
are you ending the external call string with a # ?
did you check any caveats on Rel 3.xx ? The highest you can go with 403 is a 3.xxx
did you enable trunk to trunk transfer?
are all trunks in the same group and that extension has permissions to access and dial out?
Have you run monitor to watch...
You call roll back to the previous 4 dot version or move ahead to the new December release. That should work. We had the same issue.... with 4.0.18 vmpro

Well I am wondering if SSA might give you additional info. Monitor will capture real and historical and SSA will give you alarms and real time call flow which can be hard to pick out with monitor.

I have the exact same issue with a 5620 sw ip on a vpn link back to the ipoffice 4.05. A 5610 sw ip worked fine in the same environment.
The problem is most likely the router(s). The one the client in my case has a tz70 sonic wall. There is a known issue with that router. And probably others...

If not solved yet.... ENSURE the carrier is not a voip provider....had another one today....Some cannot send or do not send forward disconnect or reversal to indicate distant caller has hung up....IPOFFICE will stay on for some time.
That is NOT an IPOFFICE problem...
