This has something to do with call forwarding which is not following your correct ARS path. With this you're not using ARS and then it all works. Some kind of design error for me! ;-)

You need to change this by using a trunk access code. This can be found if you do a "List Trunk" there you can view all available trunk access codes.

You need "coverage-after-forwarding" to be set to YES --> change system-parameters coverage-forwarding. If not set, then you call will not be covered after the forwarding to the x-ported station.
CM4 is a bit different in user administration. You need indeed to create the dadmin user by yourself. So it is not automated with the authentication file. You have to add dadmin in the "susers" group and also in an additional "prof2" group. Afterwards if an account is locked out you can unlock...
Supervisor client rel. 13 is not updated for server release 14. But you can download it at http://support.avaya.com for free. And rel 14 of supervisor client supports rel. 13 and 12 server releases! So upgrade to rel. 14 is the thing you need to do! ;-)

You can always install the default 10-user ODBC server. You can check it on you CMS if it is allready installed/running with following command:
ps -ef | grep oplrqb
and stopping and starting it with:
cd /cms/dc/odbc
./odbc_init -r 1 or 0 (0=stop, 1=start)
