Try a forced ACW of 3 seconds. This gives the PBX time to hang up the calls.
You might want to see if the people being dropped are haning the phone up the same way. We had some issues there.
IP agent V3 can only be used with release 10 or higher. If you are on 9.5 you can only have RTU's on V2. If you continue to have issues you might want to call Avaya. There are a number of patches out.
We had the same issue. You need to make sure that you are on the latest qpcn. Are you running a novel network? Are the PC's 2000? This all made a difference. Email me if you want to discuss it further.
