I noticed on your posted trace route you are using codec G711. VoIP traffic over a WAN should use G729 or G729A. Something to consider especially if you have low bandwidth.

Thanks for the programming info. The extension did not have Automatic VMS Cover - #310 - set to "assigned". After turning this on, internal VM coverage starting working.
Sounds like you have a tip & ring reversal coming from the d-marc. This will cause ground-start lines to show as "disconnected'. Try reversing the pair (jumper/cross-connect) at the port and then reset the trunk card.
Check the main line for dial-tone by doing a status of the trunk-group the line is associated with. If the line is in a "disconnect" status it could mean one of two things; there is no-dial-tone to the trunk port or it has a tip & ring reversal.
