Found the problem. The system had "Full AutoHold" enabled for some silly reason. And their system is set with Line Pool A as their prime line. So, when they lifted their handset, they were on line 8. They go to take a call on line 2 and voila! Line 8 goes on hold and blinks 'til the cows...
