Unknown IP handset - Avaya IPO R10

I see some IP Phones as Unknown IP handset in Avaya IPO R10, the problem with them is when we reboot our IPO they won't login Automatically, we need to manually enter Extension number and Security Code to Login all Extensions were programmed in same manner but only some phones behave like this. need your expert suggestion's
 
is the base extension the same as the user extension?
 
What type of phone? Where did you find the message?
 
Don't put password in user and login password at telephony try to delete user and again recreate as joe2938 said it can be extension and user mismatch issue
