IPO 500 (4.0.14) last transfer calls back

When someone using the main console transfer a call to voicemail, that persons voicemail calls back on the 2nd or 3rd call appereance. How do I stop this from happening?
 
does the transfer complete sucessfully???

Sounds like they could be putting VM on hold!

Yes the transfer completes and you can leave a voicemail
 
they do either a direct to voicemail transfer by hitting transfer #1xx or just a regular transfer by hitting transfer the ext. number and transfer again and then hanging up.
 
Check under the users Source Codes for any thing like P200.

Also make sure the VXXX is corect for the extn.

Is VM ringback turned on for these extns?

The source codes are all good and there is not VM ringback on.....they have a combination of loop start and ground start lines are there any issues with that that you know of?
