What you should do to simplify is to look at a prefix or area code that is working properly in the same switch and copy the same information...
You're opening up a can of worms and defeating the restrictions on the system. What happens is that an account code is dificult to keep secret, as users are rarely discreet when dialing. Others see the code you are using and pass it on to other restricted users...

A 4 MB card? sounds like you have a pre V9 switch...
In the Pre- V9 switches, buy the non ATA cards, as the ATA cards will not fit in the slot and are used in Version 9 and above systems...
Once you get the right card, you can do a format-mem command and it should be usable...you may have to...
