Have you put your Public ip in the network topology tab of the LAN interface? have got port forwarding on the router to enable this for your domains? I would also recommend you get a cert for security if you are going to have access remotely to handsets.
I've done it with USB upgrade. But like Sizbut says you need to go to 11.0.4.3 first before 11.1 there are centos options that need to be installed before going 11.1. Done two now with usb and both have been successful. If you still have issues with the Web would suggest you use the usb.
You shouldn't as you are already on 11. licenses should already be for 11, much like 10.0 and 10.1 you didn't need new licenses for that when upgrading.
There is a process for going to 11.1, you have to go to 11.0.4.3 first. But after that its just like normal. If you look at the upgrade doco it mentions the reasons why, i just cant remember off the top of my head.

Have you changed the login for voicemail?
There is the setting in the client Administration> General> Voicemail password
You can set this to be the same that is set in the Security settings of your IP office. You should be able to login after that.
For the digital expansion module can you see it under status? if its not there those handsets wont work that are connected to it.
Or is the expansion on a different level of firmware. they should upgrade with the system, but have seen when they dont. under status it would greyed out if different.

thats what the manager account is for. it restricts what they can change if they arent tech savvy. if they are, give them full access. if they blow it up, you can always get a fee for the fix.

use the good old ARS .sxxxxxxx options. send the 9, and 8 calls to different ARS with the options displaying the main number and the DID. Also with SIP are you using internal data or just auto? ARS option should look like:
