I found some phones that were stolen and replaced with the wrong model. I have replaced all of them with the correct one, and so far no errors. Thanks for all the help guys!

Janaya, That is a very good possibility. These phones are in an area where the users would have no regard for something like that. Many users are actually set up with 2616s because the department manager rejected the expense of getting 3903s. I bet they have stolen 3903s to replace the...

They did have a call forward loop setup. 875 forwarded to 535. 535 then sent to 303. 303 then sent to 875. Triple loop therefore broken.
