I would say that one of the people that is a member of your meeting notice has this terminated person set as a delegate. Even though the person is termed, sometimes the acct can stay in the delegation and cause this.

We use Outlook 2003 as well. Normally once you send an email to an address, grabbing the name from the address book the first time, the next time you open an email and start typing the name in the TO field, their name should pop up as an option to choose from - this is part of your nickname file.
For the first part: when the email is open, just right-click on the name and add this person to Outlook Contacts. Then open your Contact folder where you created the DL, and add your new contact as a new member.

Doesn't have to be the delegates on the actual meeting organizer's calendar - it could be delegates on any of the recipients of the meeting notice. Basically you'd have to check for these 2 delegates on each of the recipient's accounts. Seen this many times and it's always the case.
