You should using Tracing to find the issue, I had the same problem at a client, replaced all the hard still the same problem, turn out to be an issue at the Teleco Exchange.
Default Trace plus ISDN option.
I got the same problem at one of my client, what I found out was that it is a software issue and that Avaya were going on fixing the issue. The issue has not been fixed in 3.2.54.
