Can your sets forward externally already?
You have to make sure that the extension has permission to call forward outside the phone system.
Allow Redirect needs to be checked off for the extension.
I've had a similar experience to this recently.
I would start with what wMitel stated above, but my IP sets had conflicting IP addresses.
Also when replacing the phone replace the cat5e cable, I had someone roll over the cable with their chair and pinch the cable.
