Since Office 2003 is a major upgrade I would always do a clean install.
In fact, since you have 4 citrix servers I would actually take this opportunity to reinstall all the servers with a totally clean office installation, one by one(if it is possible in your environment).

You can use the Office Resource kit to create an outlook profile that maps to the new Exchange Server.
You can then provide this outlook profile to each user. There are several ways: logonscripts,outlook profilewizard etc.

You can use autocreating of the users default printers, or/and you can use citrix policies to define the printers(and settings) you want for the users.

If you are using native drivers for autocreating client printers, then this PDF printer won't autocreate if you don't install the driver on the citrix server.
if you are using the universal driver then you are out of luck -:(

Use DSMAINT CONFIG to move a SQL datastore to another SQL server.
1. Create a new SQL DSN file on each citrix server that points to the new SQL server.
2. Then use DSMAINT CONFIG on each server.

The Temporary Internet Files is located in the users profile under, \Documents and Settings\%username%\Local Settings\Temporary Internet Files, so there are no need to change this.
I think it could be the default security restrictions on Outlook attachments that's preventing some...
