Check the FRL level in your route patterns also. I don't know why but when we switched LD carriers we had trouble with some of our FRLs that worked fine before. We also had to try different setups with the digit 1 in our route patterns like dwalin mentions.
Slippage can be either or. When I had slippage on one of my DS1's and my LD carrier did testing on their end found nothing so I knew it was on my end which was a bad CSU (replaced). What type of trunks do you have?
You should have all zeros on the DS1 log. Any numbers would indicate slippage. Are the disconnects on your incoming, outgoing or both. How about local, long distance or DID?

why not try calling 1354 (group ext.) direct and see what happens without using audix. Then I would call his station direct to see if it rings normal etc. I would also change hunt after coverage to no on your coverage path. Do you possibly have both a hunt group and a cover group programmed...
Sounds like there may be something up with the second phone. Could it possibly be on call forward or send-calls? You could try (on the pbx side) list call or status sta xxxx (2nd guy) to see if SAC is activated. Also check the COR on all involved to make sure there are no restrictions to that...
If you want all phones to ring at once and anyone picks it up make your zero out extension a coverpath whos point 1 is a cover answer group (up to 8 people). Also did you list the people on the 3rd page (on mine)of your hunt group, with only one phone ringing I would check your hunt group...
Also, if the air is not fixed yet get some fans running near the equipment to help keep it cool. I know from experience because our ceiling compressor went out once and overheated the cabinets causing complete shut down or should I say melt down!

Just a guess but If it's a 302 console did you have the power supply directly wired on the wall panel like we do? If so you will need to wire a new power supply on the new wall panel.
