VM Asking for Code

IPO 4.1.9 VM Pro 4.1(27)

I dont know what I did but for some reason all voice mail boxes are asking for a "access code" when callers are transfered to Voice mail or the user doesnt answer. What heppened?

I figured it out!

For some reason one of the default start points (leave) had an action attached.
