There are two brands of IP400 PRI cards.
The first one was introduced with the Network Alchemy and this one has been replaced with a improved version and i believe that happend when IPO release 3.0 was introduced. I am not sure and right now i am too tered to search for it.
With the release of 4.0 there was a Tech Bulletin stating that the old PRI was not supported anymore.
I have seen installations running 4.1 with the old PRI without any problem. Also i have found that release 4.2 absolutely rejects these cards, it does not work at all.
I will take a photo of the old card and post it somewere so you can identify the old hardware, it is quite different as the new hardware.

I thought you had upgraded a pre 4.0 IPO400 to 4.2 but it is a IP500.
It is all in providing enough info with your question.
basically the PRI should work without a problem.
I advise you to shutdown the IPO, remove the VCM&PRI restart the system erase the PRI, reboot the system, trun it off and insert the VCM&PRI.
It should automatically identify and setup the PRI.
In monitor you will get a line like this:

The system shows if the PRI is recognized.
If not then you have a seriuos problem and replacing the PRI may be necessery.
Well thanks, and yes I left out too much. It didnt help that jgrove threw in his 2 cents and said ipo400 in the middle of this. Ive tried what you said there, and it didnt work...i wonder if there is something wrong with the card itself, although it works just fine on other SW revision. just nothing above 4.1.12. I dont really care i guess either way, i mean, its working, its just really annoying when i open up my manager and 20 some ip offices runnin all on the same SW revision, except that one. Makes me angry
