Questionairre database

How best is it to design a questionairre database. I understand i need to have a table for the questions and also the answers and then also to have a table to store the details of the people who are going to complete the database.

What is the best structure to create this from outset regrads the relationship and then to record the answers onto another table.

Any thoughts on how best to construct this as i not too sure what is best...

Ok if i was to do this how would i populate all 49 rows for each individual answer with the 'nameID' in the 'Responce_list table so when i transfer this data over to the new table that it captures this id?
 
you build a form that has as it's source a query that contains the NameID, the questions and places for the responses. When the user finishes the survey and presses a button ("Submit Survey") you run a query that inserts the information into the table including the NameID.
