Problem with tranfer & MOH 2

I have a client with an IPO 406 r4.0.7 & for 4 days now I have been experiencing a strange problem. In some ext when they transfer a call to another ext and they don't complete the transfer, but wait for the other ext to answer first, the caller being transfered hears both music on hold and ringing tone. This happens only to some ext. I cannot find any difference in the settings of these ext which have the problem. All ext in this client are digital. I rebooted the system, but it didn't help. Any ideas? Thanks
 
upgrade it, early v4 had loads of odd issues....not dtmf and much more

Upgrade the system, 4.0 releases are very buggy, go to the latest 4.2.20 ideally :)

Anything happen 4 days ago?
Not sure what you mean when you said you booted system..

But, if you didn't you can try power cycling all the modules.. Sometimes they lose their marbles and a power cycle clears it..

If not Maxwell and Amriddle are correct upgrade..

Sorry for the delay, but I was away from office. Maxwell and Amriddle were correct as an upgrade to the latest 4.2 release solved the problem. Thx
