Occasional email blast from IP Office to random user

We have an issue where when a user gets a voicemail that is set to copy or forward to email, they will continually get a copy of that voicemail every 3 minutes until we stop that service on their account.
It has happened 4 times now and to random people. Has anyone encountered this before? What did you do to fix?

Have you checked out the SMTP trace from the IP Office? Turn on SMTP tracing in Monitor while it's happening and it might give you some clues as to why it's retrying constantly
