Mobile twinning

I have a 500v2 running 10.1 software and a PRI.
If a caller dials a DID associated with an extension, calls twin just fine.
If a caller tries to direct-dial from the auto-attendant, the call does not twin.

I've performed a system upgrade from 10.0 to 10.1 and it made no difference.

Can anyone help? I've been wrestling with this for weeks.

VM Pro or embedded?
Assisted (VMPro),"Normal Transfer"(EmbeddedVM) or a Transfer(VMPro&Embedded)?
Does a blind manual transfer from another extension works?
What does monitor tells you?

Blind transfer does work from another extension

FYI...Extensions begin with 6 (600 range)
In Auto Attendant, "dial direct by number" is checked (active)
In the "actions", option 6 is defined as "dial by number
 
Did you use monitor to observe what the difference is between a manual blind transfer and a dial by number throughthe EVM?
 
I've had very little experience with Monitor. While I can log in and observe the system activity, I'm not sure how to set up the filters to focus on what we want to see. Suggestions?

Set to defaults and then post here we can help. The best thing to do is test when you will likely be the only call trying so that we don't have to sift through a bunch of different info. The best test would be to make a working call then a call you know fails to the same extension.
