Mitel SX-200 EL ONS Cards locking up

We have a client with an old EL system and we have a wierd problem with ONS cards locking out.
Cards in slots Bay 1
1 - DNIC
2 - BNIC
3 - BONS
4 - BONS
5 - BONS
6 - BONS
7 - ONS

Bay 2
1 - ONS CLASS
2 - ONS
3 - ONS CLASS
4 - ONS CLASS
5 - ONS CLASS
6 - ONS

If a user calls out on any extension in slot 7, the call will complete but then all ports on the card go into dialing, then wait for ...., error, then lockout. No leds are lit on the card and no logs in the alarms. System has been reset, cards have been moved around. Only way to clear the lockout is by reseating the card in slot 7.

Any ideas folks?
