Mailbox already in use

I've got a receptionist who's having an odd problem. When she forwards a copy of a voicemail to another extension, she isn't given options to delete the message, reply, etc. She hangs up and then goes back into her voicemail to delete the message, but gets this error: "This mailbox is already in use, please try again later. Exiting the system, goodbye."

Thoughts on how to resolve this problem?

What is her extn number and mailbox number
What is the extn you are delivering mail to and its mailbox number.
 
Her extension and mailbox are 3000. She delivers the mail to several 30xx extensions that have corresponding mailboxes.
 
is she sending the message to a group mailbox which she is a member..group mailboxes use 901.902 etc.
Its possible that she is sending the message back to herself thus engaging her mailbox because its in use to send the message,

Use BCM monitor and look at the voicemail ports to see how many are in use when the message is being sent.
There may not be enough mailbox ports available at this momment

I Meant to ask is it a bcm50 which has 10 ports or older bcm 3.6,3.7 which may only have 6 ports available.

increase the mailbox ports
 
She is not sending to a group mailbox.

This is a BCM 200, 3.5 Minimum of 2 voicemail ACD ports, maximum of 6.
